Your student is required to have the following items and associated fees completed prior to orientation. Use this page as your checklist to make sure you have all items completed.
- Please read our Student and Parent Handbook carefully as our policies have been updated. An electronic signature is required stating that you understand and accept all policies and procedures as set forth in the St. Thomas More Catholic High School Student Handbook. Your electronic signature will be captured in the Policies Permission Form below.

- Parent Release Form for OTC Medications | This form must be completed for all students.
- Parent Release Form for Prescription Medication | Print, complete and returned signed form only if your student needs prescription medication during school hours.
- Allergy Questionnaire | Complete only for students needing an Epi-Pen during school hours or after-school activities.
- Asthma Questionnaire | Complete only for students needing an inhaler during school hours.
